Evaluating how well a diagnostic test identifies disease at a single point in time is best done with a prospective cross-sectional design. Prospective means you plan the study and collect data going forward, applying both the new test and the reference standard to participants from the same cohort around the same time, and recording results without leaning on past records. This approach reduces biases that can creep in when looking back at old data and helps ensure consistent test administration and blinded interpretation of the reference standard.

Cross-sectional means you assess disease status and test results at about the same time, which is ideal for estimating diagnostic accuracy metrics like sensitivity and specificity. You don’t need follow-up to see how outcomes unfold, which would be the realm of longitudinal designs. Longitudinal designs are more appropriate for prognostic questions or when you want to see how test results predict future events, not for determining instantaneous diagnostic performance.

Retrospective designs, in contrast, rely on existing records and can introduce biases related to how data were collected or recorded, and the timing between the index test and the reference standard may be uncertain. Therefore, they are less ideal for measuring true diagnostic accuracy.
